Für Bewerber um den Karl-Sczuka-Preis (falls sich mehrere Künstler oder ein Realisationsteam bewerben, bitte für jeden Bewerber ein gesondertes Formular beifügen).

Applicant for the Karl-Sczuka-Prize (for cooperating artists and production teams, each individual should fill in a separate entry form)

Please fill in the entry form completely. You will recieve a copy of the form in your mail account. Please print it out, sign it, and send it together with the following documents to the Karl-Sczuka-Prize
1. A manuscript or a synopsis (six copies)
2. A short biography of the artist (six copies)
3. A list of all works of the artist (six copies)
4. Recording of the production ready for broadcast (one copy)

Bitte füllen Sie das Formular vollständig aus! Eine Kopie des Formulars wird an Ihre Mailadresse verschickt. Drucken Sie diese aus verschicken Sie diese unterschrieben zusammen mit den folgenden Unterlagen an den Karl-Sczuka-Preis:
